Here is a copy of the New Furnace Waiver Request Form as required by IHWAP. Open it up, fill it in, and it will automatically calculate the cost for you, Walla! Oh, and I changed it to standard 8.5" x 11" the original email form was an 8.5" x 14" - Link below form picture.
